  2. Fill out a New Client Form (if you're a new client).

  3. E-mail records to fwmobilevet@gmail.com 24 hours before your appointment.

  4. Someone from Fort Wayne Mobile Veterinary Services will contact you within the day to schedule an appointment. If you do not hear from them within 24 hours, please call to ensure your new client form was submitted properly.

  5. Once an appointment has been booked, please pay for the required preliminary fees (travel and +/- exam)

  6. Dr. Molly will call the night before to collect history information, payment for the preliminary fees if needed and to give a more specific time of arrival.

  7. The day of the appointment, Dr. Molly will call once she has left her previous appointment to let you know when she will arrive.

  3. Sometimes euthanasias are planned, sometimes they are not. If this is something that needs to be done today, please also call Dr. Molly in case she is busy in an appointment and doesn’t see the new client form right away.

  4. Dr. Molly will send you the euthanasia forms to fill out on the computer. The euthanasia form gives us the legal information we need to continue booking the appointment.  The cremation portion allows you to decide if you would like ashes returned to you or cremated with other animals and no ashes will be returned. There are also memorial keepsakes you can choose to purchase at this step also.

Dr. Molly, our founder, is stepping aside for a short while to welcome her new baby into the world.


Fort Wayne Mobile Veterinary Services will pause services during Dr. Molly's maternity leave, but will resume as soon as possible!
